Username Changer

I don't think I have seen this one so here goes...

It would be nice to have a mod that users can request to have their name change and it goes into some kind of name change queue and you can approve/disapprove the name change request.

I've got a username changing add-on, but it doesn't use an approve/disapprove method, it just allows them to change it.

That's released right? It probably wouldn't be hard to customize it if we extended on the moderation queue right?
