I thought that was intentional as before if there was an image with no text, nothing appeared in the overlay.

Yes, this is the designed behavior - it is stripped, that's what it's replaced with (avoids language issues to some degree).

For the thread preview, there's an option inside this addon (screenshot)but it will still display the content of any bbcodes (so the attachement id for attach bbcode).
