Template for Posts

Hi, is it possible to give the user a template when starting a new subject? For example, if it was a forum on hotels I'd like a template that would be something like this:

A heading with:
Hotel Name - City - Address - Photo

and then separate subjects that user could comment on such as:
Service
Cleanliness
Room Size
Access

Like I said it would have to follow the specific template of Hotel details first, then the pre-defined subjects, which other users could then comment on as per usual.

In the default software, no, this is not possible. However, it can be accomplished via an add-on.

I do not believe that add-on has been created. @Waindigo may have something you can use to accomplish, but I do not believe I have seen exactly what you are looking for.
