Add-on Server Application Addon

I run aminecraft server which has a whitelist.

I currently have users submit applications via forum thread.

Is there an addon out already what would allow me to make a set app and then have someone fill it out and then just approve it deny it when its done?

Not sure if this makes sense if not just ignore it haha
