Paid subscriptions is built-in (known as Paid User Upgrades). You can use this feature and give people a special usergroup, acting as a "paid-only" forum.

Yes, only Paypal.
There were addons listed here a while ago for Moneybookers and Authorize.net integration but they have been removed by the author.
