MySQLi: Duplicate Entry "63-3275" for Key 1.

One of the admins on my forums got this error when he made a post on my site and accidently made a double post but hit the sumbit button only once. Here's a cropped version only showing the error:



Do you know what caused this? Not sure if this could be because I'm using an older Xenforo (I don't know how to properly update it without breaking something and I'm afraid to) which I think maybe 1.0.1... Would it be because of that? Or just some fluke?

Translated into English... that error says the user tried to watch the thread but he is already watching the thread. This is perhaps some kind of race condition relating to the double post.
