Fixed Missing index on purchase_request_key for active/expired user upgrades

The column xf_user_upgrade_active.purchase_request_key/xf_user_upgrade_expired.purchase_request_key is used to locate purchase requests in a number of payment providers, but this column doesn't have an index resulting in slower than expected queries.
