Duplicate JS issue when responding to reviews

I think there may be a bug when replying to a review.

When I click the post reply button, the page doesn't update, there is however an error in the console, if I then refresh the page the reply had been successfully saved despite the error.

Can't really test easily myself on this site to reproduce as I can only reply once.

Thread 'Viewing reviews as the resource owner throws javascript errors'

When viewing the reviews tab for a resource I own, the following error is thrown:

After clicking reply to a review the reply posts but doesn't do the post submit convert to html, and throws this error

There is more debugging on this, and yes replying to reviews is consistently buggy :(
