XF 2.2 Headlines in post body do not get rendered reliably

For quite some time I do have the issue that headlines do not get rendered reliably. Most of the time it works as it should, but sometimes it does not. This does also affect older posts and articles where it used to work fine. It started out of the blue and does not affect all pages/threads/articles but only some - however, I was not able to discover a pattern until now. Looks like that then:

With this two year old article for example rendering used to work fine, even after the trouble started. Only recently the rendering got bonkers.
When it started it was typically after editing an existing article or post which had headlines in it. When saving a edited post there would be additional BB-Codes for the headline rendered after saving aside of the existing headline (that itself still rendered correctly). Looked like that then:

These would be visible when editing the article again (and could w/o harm be deleted). Also, when copying over formatted text including i.e. headlines from other websites allignment tags like "center" would be copied as well and redered as BB-Code after saving. Had not happend before.As there was a coincidence that this started shortly after I installed the add on "Advanced BbCode pack" by @Xon (in the meantime updated to the actual v1.15.14) I had the suspicion that this add on may have caused the issue somehow, but then I had a number of other add ons installed during that timeframe as well, so no real prove and did not bother to file a proper bug report with Xon back then. However, due to posting in the support thread of the add on back then I know, that it occured first around a year ago.

I ignored the issue for quite a while as it did not happen too often, only somewhat randomly and only after an existing post had been edited. However, in the meantime it has become worse, it shows up more often recently and some headlines now have lost their formatting completely. Even in the same article there are headlines with and w/o formatting:

As this shows now pretty regulary throughout my forum I am fed up with it and trying to find the root cause. I disabled Xon's add on but this did not bring back the correct rendering - headlines would still not show as headlines despite having the correct BB-Codes.

However, when editing such a post/article and switching to BB-Mode it does look completely weird:

It is clear as water that I did not manually add in the bold and the size. Could swear on it - but in the end I cannot be absolutely absolutely sure that I did not things like that as a fix, but 99,9% sure. So something nasty is going on here.

I still suspect an add on or a conflict between two add ons as the root cause but do not have any idea how to track this down.

Any hints?


PS: When I look at my installation log of that timeframe this is what was installed or updated before the trouble started:

These are custom bb-codes added by my Advanced BbCode pack add-on. Disabling the add-on may because all sorts of rendering issues if the bb-code itself is still enabled.

Make sure you have the header bb-codes installed from Advanced BbCode pack. They are custom bb-codes so you should be able to check the admincp bb-codes list to see if they are installed

This was a hint in the right direction. The BB-codes were there, but (for whatever reason) the ones for headlines were disabled:

I rebuilt the add on, nothing changed. I manually enabled the custom BB-codes for h1-h4 and now rendering looks as it should (at least on the article that I used to test the issue). Time will tell if this is persistent throughout the forum and also, if it will work with new posts, too.

I've no idea why those were disabled, can hardly imagine to have done this - but who knows.
