Fixed Follower user stuck following its own after the accounts merged

When a user has multiple accounts and follows one account from the other one, stuck "following its own account" after the merge proccess.

Change log:

Ensure a user cannot be following or ignoring themselves as a result of a merge, and rebuild following caches correctly.

Any changes made as a result of this issue being resolved may not be rolled out here until later.
