I am looking for a Contact Form which i can add custom fields for members to fill out and maybe even the ability to create forum topics when a new form is completed.

Hi ragtek, I saw your thread but I dont think it has some features included.

Im looking for a contact form that has a drop down menu which the admin can add departments too.

Advertising
Complaints

An option to specfy what email address the emails go through too eg:

Registration (members@mysite.com)
Advertising (customer@mysite.com)
Complaints (complaints@mysite.com)

Or they all go through to the same email address but have the department in the subject box.

Hope that makes sense!

This is a similar type form im using on IPB. If logged in it fills in your name/email address with those on file.
