XF 2.2 Attachments upload but show as tiny square

Please see the attached image. When I click thumbnail or full image it adds another square to the post body. Any ideas how to fix? Thanks in advance!

Have you tested on default style with addons disabled?

yes it still happens in default style.
only addons installed are xmg and xf importer

attachment settings are as follows:

The attachment max height is set to 5 pixels.
