XF 2.1 One User Can't Attach Files

I've got one member who recently found himself unable to attach images to his posts. The images upload in the editor but appear greyed out with a strikethrough after the status bar finishes. As a test, he tried re-posting an image he was able to post a while ago but encountered the same problem.

I was able to post two test images with my account without issue. I checked his account settings against another regular user and they're identical.

Any ideas?

Yeah, turns out my site wasn't the only one he was experiencing problems with. He recently upgraded to Win10 and has had issues ever since.

I'm doing some more testing now, but in case anyone in the future comes across this thread, it looks like my issue is with modsecurity.
