XF 2.1 Media uploaded but not post-able, shown with strikethrough?

This issue has arisen now on two of my sites but let's stick with the first one.
Power user 50k messages. Uploaded tons of media. Says now all of a sudden his photos upload but are not post-able. They show up under his post as if uploaded but with a strikethrough and without the ability to add to the post.
Server has lots of space. No settings have recently changed. User states devices are same and issue is replicable across all.

Check post_max_size and upload_max_filesize in php.ini.

Thank you guys. It was post max and upload max. Parameters reset by a recent upgrade

Ok so re-topping this because the person on the other site - same server - is having this issue still.
And it's been quite popular as an add-on feature for premium membership so it's rather high priority.

My site early on tripped “ModSecurity” for some strange reason. Hosting company sorted it out rather quick. Certainly worth looking at if you’ve exhausted other options.
