XF 2.1 New members not in Approval Queue

Sometimes I find new members appear in my list of members (Sorted by Join Date) that did not appear in my Approval Queue.
Does anyone know what might cause this.
It worries me, as I can not trust my Approval Queue, and I might miss out on a new member, unless I check the list of users two or three times a day

Check their 'User state' to see if they have confirmed their email. This happens when members register but do not confirm their email. Once they do, it will appear in your AQ.
