XF 1.3 cannot approve member

Hi my system has blocked a user from being able to register without my approval, i then approve and then a message stating

Error
Please enter a value for the required field 'username'.

pops up, im unsure what to do as i cant allow the new user access,

Does it happen with all add-ons disabled?

Try rebuilding the master data by navigating to the /install path.
